CHANGE OF PLANS When Los Santos crime boss Carlo Pescador was set to testify before the grand jury, Carisi contacted Stabler. Irritated, Carisi said over the phone, "Come down here immediately; Bryanna Pescador has taken back her testimony.” Given that Carlo’s sister serves as the primary witness against him, Stabler believes that Carlo might have coerced someone to influence her change of heart. However, without concrete proof, this remains speculation. When Carlo requests a meeting with Stabler at the holding facility, he appears sincere as he claims an unknown individual, aligned with what he refers to as a "new player" who disregards all norms, is behind pressuring his sister. Carlo cautions Stabler that unless these issues get resolved soon, a fresh wave of drug-related conflict could erupt. Subsequently, despite Carisi pleading with the judge for more time to gather additional evidence, Pescador's case gets dropped due to insufficient proof.

When Carlo is embracing his attorney and getting ready to leave the courthouse as a free individual, the bailiff(!) moves forward and fires several shots at him, hitting him in the chest and causing his death. Stabler and Bell draw their guns and chase after the bailiff, eventually trapping him in the corridor. Stabler commands him multiple times to let go of his weapon. The man shouts back, “I can’t!” before Stabler pulls the trigger. Following procedure, Bell confiscates Stabler’s firearm afterward. They then discover a Catholic prayer card in the pockets of the officer, with certain words having red circles around some of their letters.

THE ITALY YEARS The card triggers a flashback for Stabler to Naples, Italy, from six years prior, where he apprehended a woman named Isabella Spezzano (portrayed by Blindspot Mary Elizabeth Mastrantonio portrays a character who stabbed her husband to death. Initially, Isabella resists Detective Stabler’s questioning; she ridicules his accent and remains tight-lipped regarding her involvement with one of the region's most prominent criminal organizations.

Stabler video chats with Kathy (hello, Isabel Gillies!) since he’s going to skip dinner. She acknowledges this understandingly, and their warm chat afterwards offers insight into where their relationship stands now. We’ve heard previously that the Stablers appear to have improved significantly as partners compared to during his tenure at the Special Victims Unit. They exchange affectionate gestures like blowing kisses. She recommends feeding Isabella something delicious. He peppers the conversation with some Italian expressions of fondness. Meanwhile—just so you know! —this scene takes place after Benson’s departure. William Lewis , roughly Rob Miller era.)

When Stabler offers Isabella some of Kathy's spaghetti, she is quite pleased. They end up discussing her grandchildren, who mean everything to her and cause her great concern. Stabler suggests that Isabella could help take down the Spezzano crime family by cooperating with them, allowing her to reunite with her grandchildren. Her wish is to have a horse farm, something he cannot promise her, though. can Promise her liberty in the USA. She accepts the proposition.

OLD FRIENDS, NEW TERRITORY In today’s timeline, the NYPD's Internal Affairs Bureau quickly exonerates Stabler; Bell promptly returns his weapon and badge, allowing him to return to duty right as Reyes reports that the individual Stabler shot murdered the bailiff and took his uniform and identification to infiltrate the courthouse unnoticed. While they remain uncertain about who specifically targeted Pescador for death, Stabler recognizes the prayer card from his days in Italy—it serves as a communication tool used by an Italian organized crime group akin to the mafia known as the Camorra, which disseminates instructions to its operatives. The situation escalates further when Bryanna informs the Organized Crime Control Bureau that her siblings—who had ties with Los Santos—were assassinated through a car explosion in the Dominican Republic.

Stabler goes to see Isabella, who is doing well for herself with a big estate and a thriving horse farm, which is "all legal," she reassures him after greeting him warmly. After pleasantries - which involve her condolences about Kathy - he shows her the prayer card. "Is the Camorra here?" he asks. She says she doesn't know and definitely isn't involved in anything unsavory, but she agrees to help him decode the prayer card.

In the meantime, Bryanna Pescador is murdered by a shooter on a motorcycle as she walks down the street with her adolescent daughter.

THE NEW PLAYER UNMASKED When the squad identifies a potential lead on the identity of the new shooter, Stabler and Reyes head over to speak with the suspect’s grandmother. During their journey, Reyes confides in Stabler that he recently broke things off with Jet—a couple of weeks since she was wrapped up in the trucker investigation—and expresses his concern for her well-being. This worry resonates with Stabler as well.

The trip to see grandma takes a dark turn when the shooter—a teenager much like the impostor deputy—climbs onto the rooftop and chooses to leap to his death instead of giving himself up to law enforcement. It later becomes clear that both young men had been employed by the Camorra, an organization known for enforcing unwavering commitment under threat of harsh repercussions. Detectives Stabler and Reyes discover yet another prayer card hidden in the second youth’s room and deduce that the Camorra has begun enlisting members within the U.S.

At Isabella’s place, she reveals to her grown-up grandson, Roman, that the Camorra have arrived in America and plan to target her due to her betrayal of her brother, Rocco. Roman insists they won’t be able to harm her. She resumes deciphering messages and quickly verifies with Stabler that the playing cards contained directives for eliminating Bryanna and Carlo Pescador. The Camorra bribed Bryanna to retract her statement so that Carlo could either walk free or become more vulnerable as a target; afterward, they eliminated her to eliminate a potential witness.

When OCB tracks the funds, they discover that it’s linked to a shell corporation which is also associated with a recently opened brewery in Long Island City... operated by Roman, who turns out to be much more involved in the family business than anybody, even Isabella, had understood.

JET JETS Sure, let’s revisit Jet. So here’s what’s happening: Bell goes to visit the hacker at her place and discovers that the serial killer-trucker case continues to weigh heavily on her mind. She has developed an advanced database specifically designed to locate potential victims and has pinpointed over a hundred of them so far. Ayanna mentions that the FBI extended a full-time position offer to Jet. “I declined,” Jet responds hastily. However, Bell assures her that should she wish to transition away from the OCB, everything will work out fine. After all, as Sergeant Stabler requested assistance earlier with a favor, he adds, “you’ve contributed four valuable years.” Hence, Bell concludes, “do whatever feels right.”

Later, Bell informs Stabler that Jet will be taking an indefinite leave since "she's pursuing her personal path." It’s just him at the office when Jet stops by to collect her belongings. He gives her a hug and mentions how much he’ll miss her. Afterward, he offers to drive her to the bar where everyone else is gathered to say their goodbyes—partly because he wants to ensure she makes it there safely. Has she left permanently? ?)

EVERYONE DOWN! In this episode, there isn’t much drama involving the Stabler family... at least not until near the end. Elliott and Eli have some minor disagreements: initially, when the elder accuses his son of inadequately bidding farewell to Becky earlier that day; later, when Elliott’s interrogation of Eli’s field instructor leads to mild hazing from Eli’s fellow officers.

This situation creates tension primarily between Randall and Elliott, as Randall collaborates with Bell to arrange an MRI for El due to concerns about potential signs of a traumatic brain injury. Elliott feels upset because he believes Randall is improperly handling the role of a caretaker. Meanwhile, Randall resents what he perceives as Elliott’s casual attitude toward such serious health issues. Additionally, they talk about how frequently Bernie mentions receiving calls from someone named Joey. Initially dismissing these claims, Randall finally pays attention during another interaction; specifically, when arranging Bernie’s bedding and notices she has received a call from an unknown number on her phone. Picking up the device, he says tentatively into the receiver, “Joey?” Only silence greets him before the connection abruptly ends.

Later on, Elliot arrives home from Jet’s party coinciding with Eli and Becky returning to their apartment. All of a sudden, El notices a motorcycle speeding towards them and yells for everybody to hit the deck moments before a gunman opens fire on the group, cutting the scene to black immediately after.
